  • A decision log that captures what was decided, why, and what alternatives were considered

Where it breaks: Decisions live in meeting notes or slide comments. When the final deliverable is assembled, someone reconstructs the rationale from memory. The deck doesn't reflect the actual discovery process, it reflects what the author remembered to include.
